Rhinoplasty Surgery Advances With the FDA Release of a New Flexible and Absorbable Scaffold for Plastic Surgeons

Sacramento California. One of the most exciting developments this summer has been the release of a product that is designed to help plastic surgeons with nasal surgery or rhinoplasty. A resorbable polydioxanone plate (PDS – a synthetic bio-absorbable material) developed by Ethicon, Inc., recently approved by the U.S. Food and Drug Administration, which serves as a scaffold for cartilage reconstruction. This product has been available in Europe and much of what we have learned about the product comes from our European colleges. We are excited to have this product available to help you achieve your goals.

The PDS Plate For Rhinoplasty

The Flexible and absorbable PDS plate assists your rhinoplasty surgeon in achieving a straighter nose following trauma, nasal fracture. It has also been utilized to assist in strengthen the cartilage grafts sometimes required to make a pretty nasal tip or when removing large humps from the bridge of the nose. The septal or ear cartilage grafts utilized by your rhinoplasty surgeon when creating your new nasal contour can be secured or fixed to the plate allowing for greater strength – like a batten. This greater strength may allow for improved straightening in some cases. The plate has even been used as a replacement for the septal cartilage if the septum is unusable following nasal fractures. The ability of the plate to assist the nose heal predictably and then dissolve over time is a real advantage for nasal surgery. We are excited to have yet another tool to assist the plastic surgeon in achieving predictable and reliable results.

Revision Rhinoplasty: Shaping of the nose with fillers in Sacramento California

One of the exciting new ways of shaping the nose is through the use of fillers. Dr Charles Perry of Chrysalis Cosmetics located in Sacramento California is finding that Radiesse and Juvederm work very well in the nose to enhance and improve the contours. Following a rhinoplasty procedure there can be some minor contour irregularities that result from the natural healing process. These subtle irregularities are often diminished with the volume augmentation from filler (Juvederm or Radiesse) in as little as an hour consultation with an injection done right in office. For many this is a perfect way to enhance their appearance and feel better about a subtle irregularity. Rhinoplasty revision has never been easier. Certainly Radiesse or Juvederm are not right for everyone and using these products in the nose is not FDA approved at this time. In some cases some patients will choose to have an injection over having a rhinoplasty to see what it would look like to not have a bump or to have a more balanced nose. Augmentation Rhinoplasty in Sacramento, Folsom, Roseville and Stockton

Rhinoplasty has become one of the most common procedures in Sacramento California as more and more people are realizing the benefits of nasal shaping on their appearance. In the Sacramento Valley Plastic surgeons such as Dr Charles Perry are finding that standard reduction rhinoplasty is not for everyone. On occasion some noses will require a little augmentation to the bridge or dorsum in order to make the nose appear more defined and beautiful. The Closed Rhinoplasty in Sacramento for a Twisted or Crooked Nose

New Techniques in Rhinoplasty Allow for An Individualized Approach to Your Nose

Rhinoplasty has become a much more predictable operation over the past few years as plastic surgeons share their information with one another and our documentation of previous results allows more thorough analysis. The advent of computer imagining has also allowed us to be more precise with communicating our goals and ideas. One of the new trends in nose shaping surgery is the addition of structural support and cartilage suturing techniques to alter and enhance the shape. At Chrysalis Cosmetics we believe that this can be done well through both an open rhinoplasty approach as well as a closed rhinoplasty approach to the underlying structures of the nose. As a Plastic Surgeon specializing in Cosmetic Plastic Surgery Dr Charles Perry feels that the approach is not as important as the technique. Structural modification and enhancement of the nose seems to give a more predictable result. This is different from years past when resection and shrinkage was the norm. While great results were achieved sometimes the healing process did not proceed as planned which can then require a rhinoplasty revision. One of the most difficult aspects of rhinoplasty is predicting how the skin will shrink after the surgery. Everyone heals so uniquely that results can vary based upon this factor alone. While the nasal cartilage and bone can be adjusted the nasal skin or coverage has to heal to the new shape and this process is controlled by your body. When the skin doesn’t shrink up tightly to the bones and cartilage the nose can look ill-defined and not give the well shaped look expected by some patients. This is one of the reasons that rhinoplasty has evolved over the years as plastic surgeons look for more predictable better results. We have found, like other plastic surgeons that sometimes rhinoplasty can be made more predictable when we add structure to the nose and stretch the existing skin. We are seeing more balanced natural appearing nose now using this technique of removing a hump and adding some tip or vice versa. As we grow more comfortable making more balanced noses the pendulum has switched to using more open techniques and sutures to enhance the nose. While often more predictable we do still occasionally see a few noses that look operated on as the healing process over shrunk the skin over the sutured and grafted cartilages. One way to avoid this is using additional tissue to cover these irregularities and this seems to allow for a more natural less operated look. While this is often achieved with a combination of resection of some structures, the addition of a few sutures and some cartilage grafts to enhance the shape. We are also using more injectible fillers to contour the nose specifically the bridge and tip. (Below befre and after photos of a non-surgical rhinoplasty)

Rhinoplasty in Sacramento California

Surgery of the nose is a delicate and precise procedure that requires a certain acumen or knowledge. Dr Charles Perry of Chrysalis Cosmetics has devoted a large portion of his practice to rhinoplasty surgery and prefers the endonasal rhinoplasty or closed rhinoplasty approach to nasal reshaping. Dr Perry usually performs the rhinoplasty procedure under general anesthesia as he feels more comfortable with the safety of the anesthetist in control of your airway and with you asleep and comfortable as the procedure can take up to 2 and a half hours to complete.

At Chrysalis Cosmetics your nose is evaluated on the outside and the inside during your consultation for endonasal rhinoplasty. Pictures are taken of the nose and potential results can be reviewed. The procedure and the recover following your nasal shaping will be reviewed with you by the staff as well as Dr Perry. In general most people take a week out of their lives to heal. This allows for most of the swelling to resolve and the occasional bruising around the eyes to subside. The newly formed nasal bone and shapely nasal cartilages are allowed to heal as the swelling resolves. The shape of the nose will become more formed as time passes and the results even more dramatic in most cases.

Revision Rhinoplasty

Rhinoplasty can help improve your facial balance and beauty uniquely. Plastic surgery of the face requires delicate and precise changes that are often subtle yet offer dramatic enhancement. Most often a primary rhinoplasty procedure results in a beautiful enhancement. Occasionally (<15% in most big studies) a secondary or additional plastic surgery of the nose is required to achieve the desired enhancement. The secondary or revision rhinoplasty procedure be required for a variety of reasons. Often it is due to the healing process or even a change in expectations. Once your nose has been operated on it can undergo some unpredictable changes in the healing process. Following rhinoplasty the nose swells hiding many of the subtle changes made during the rhinoplasty. This swelling can also lead to collagen deposition that is unpredictable and can alter the nasal shape slightly. During the healing process any trauma to the nose can dramatically alter the nose as well. Because of these and other causes revision rhinoplasty is often required to achieve the look and function you desire.

Revision rhinoplasty is usually done through a closed approach using hidden incisions inside the nose. Through these internal tiny incisions the bump can be reduced more, the internal and external airways opened and contour irregularities of the sides improved. The revision or secondary rhinoplasty surgery is often much more difficult as the tissue of the nose have been altered. This makes their healing less predictable and somewhat more challenging to enhance. It is often best to have the revision operation done by the surgeon whom performed the primary surgery as they will know the nuances of the previous operation and also are familiar with your tissues ability to respond to changes.

Dr Charles Perry performs revision rhinoplasties in the fully accreditied surgical suite in the office. He prefers to wait a year following the original rhinoplasty before proceeding with a revision. This allows all of the tissue to heal and allows for the additional contraction and shrinking of the nasal skin to occur. Waiting the full year allows for other subtleties to appear and be revised as apposed to operating too early and having to revise the revision rhinoplasty. While many patients are good candidates for a surgical procedure that lasts about three hours, Rhinoplasty may not be necessary in all situations. In fact some patients have found filler injections to work well for nasal dorsum volume additions and for small radix and columellar base augmentations. Similar to lip enhancement with Restylane, nasal dorsal augmentation can be performed with Juvederm Ultra and Juvederm Ultra Plus. Silicone, Gortex as well as bone or cartilage are still being used to surgically enhance the dorsal profile of the nose by many plastic surgeons as alternatives as well. Rhinoplasty (Nasal Tip Surgery) Stockton California

When evaluating the nose for rhinoplasty Dr Charles Perry of Sacramento California will assess the tip of the nose in relation to the rest of the nose face in order to create harmony bring out the inner facial aesthetic beauty. Nasal tip surgery can be done through an open rhinoplasty approach which allows for more visualization and different approaches to optimize the look. The time it takes for the swelling to resolve is slightly longer than the closed or endonasal surgical option.

This is why Dr Perry and other Plastic Surgeons in the Sacramento area often utilize a closed approach to nasal tip surgery. The closed approach is often referred to as the endonasal rhinoplasty surgery and is performed at Chrysalis Cosmetics by Dr Perry. Healing usually takes between two weeks to three months and best results are seen at a year following surgery. Closed Rhinoplasty Surgery In Sacramento California

Closed rhinoplasty in Sacramento California is often used to reduce the bump on the bridge of nose. It can be done to make the tip and base of the nose more refined and improve the nasal aesthetics. Tiny incisions are made on the inside of the nose that allows the surgeon (Dr Charles Perry of Chrysalis Cosmetics) to remove excessive cartilage which improves the fullness and lifts the tip.

This reduction of fullness can help shape the nose. The internal nasal valve and external nasal valve can then be supported with additional cartilage and sutures to allow for improved airflow as well. The septum or midline supporting structure will also be straightened and shaped as well. It is truly amazing to see what can be done through such small little incisions. It is wonderful to see what a balanced nose will do to harmonize and beautify the face. Dr Perry will remove the nasal splints a week after surgery and most patients in the Sacramento Region go back to work after the cosmetic surgery in a week. Full healing does take a year following rhinoplasty or nasal surgery and revision rhinoplasties are not commonly performed until this time.
