Sacramento California. Over the course of a typical day at Chrysalis Cosmetics, I consistently feel gratitude for the amazing people we meet. It is a constant reminder that plastic surgery is more than just a field of medicine for helping the ill or sick. Moreover it is a tool people use to make the positive internal changes in their lives visible to those for whom they care. It is most often done in conjunction with an internal awareness of self that needs to be expressed. I am so thankful to be in the unique position of assisting in a small way to such positive physical enhancements for those that select Chrysalis as their vehicle to make this expression.
An example of a recent reminder was being told a story of a woman who had a tummy tuck and has been finding that everyone thinks she has lost weight. Another woman recently completed healing from a facelift and laser resurfacing, started a new career and continues to look 20 years younger – her friends are envious, she relates with a smile. Another reminder was a young woman who had her stomach muscles repaired, and now feels comfortable training for marathons. There are many others and each one unique and important. It is this excitement and hope that creates such gratitude in our office.
The stories we hear and get to share in are numerous and inspiring. I think that is why plastic surgery is a field of gratitude. I feel so lucky to be a part of so much joy. Change is possible when we let go of our fears and trust in those who have gone before us on this path of transformation. Change is never good or bad, black or white, it is just that – change. We make it good or bad, happy and vital if we choose. Life is so full of choices and as a plastic surgeon I am surrounded by those who continue to make decisions to enhance and improve their lives, careers and families and friends. For this I am truly grateful.

It is interesting to see so many Americans traveling abroad to have their cosmetic surgeries performed. It is also troubling to read blogs encouraging people to travel for their surgery citing one example of a succesful outcome. Its like knowning someone who won the lottery. We certainly realize that it is cheaper to have plastic surgery in other countries, however, cheaper is not always better and the risks you take may be quite high. The real question is will the cost savings (often less than a $1000) be worth the hassle and extraordinary risks as well as the travel arrangements and extra time away from home?

Only you can answer that. Let us consider why staying close to home in a familiar and safe setting with board trained physicians may actually be worth a few extra dollars especially when it comes to your health and body. Things do not always go as planned. In plastic surgery as with life there are so many variables to account for that occasionally things happen that are unforeseen. At Chrysalis Cosmetics in Sacramento we strive to account for this by discussing the possible unexpected outcomes prior to your procedure. We also have a written form that again goes over the possible outcomes that are unintended as well. One of the other ways that we suggest as well is to view the procedure on video prior to undergoing your consult so that you can ask questions and feel comfortable with the process and the cosmetic procedure you would like. One of the sites we suggest visiting is the ASPS animations of your procedure. This site will explain and allow you to view your procedure in detail as well. It is important know that we do everything in our power to keep you safe during your transformation process and will help guide you along the way. On occasion a revision surgery may be necessary to optimize the result and account for some unexpected healing difficulties. It is better to plan for these in advance than to be surprised by this afterwards as there is often a cost associated with the revisionary surgery. It is important to remember that you are paying for a cosmetic procedure not a result. And that the result and aging process are fluid over time. In our opinion yes.
A cosmetic and plastic surgery practices combines both artistry and disciplined medicine in obtaining a beautiful result. Plastic surgeons learn this through an extensive time in residency and continued learning once their training is complete. This required training, monitoring, continued learning, and continued self critique as well as processing that others allows all plastic surgeons the opportunity to grow and become better over time. The enthusiasm and desire for excellence your plastic surgeon demonstrates go without saying as this is often a requirement of attending a residency program in plastic surgery.
But why is it necessary for your plastic surgeon to operate in their own operating room? Why is it necessary for them to maintain all of the expensive equipment and meet accreditation standards every year? The simple answer would be convenience and patient comfort. It is true that this does allow the surgeon to utilize his time and his staff’s time more efficiently in some regards. It is also a more discrete environment for the patient and the focus is solely upon the singular patient. But the answer is deeper than that. It begins with the desire to create and enhance. A process begins in the drawing stages in the plastic surgeons office. It is here that plans are made. As if an architect the surgeon and patient create a plan to achieve an enhancement. Preparations are made and materials are secured by the staff whom known the patient intimately. The operating room and staff are stocked checked and prepared with the individual patient in mind. The cleanliness is confirmed and the materials double checked. The plastic surgeon and the patient enter the theater feeling calm and reassured that the whole team involved in the procedure are on the same page and have the same goals in mind. This allows the surgeon and the nurse as well as the surgical technician to perform their duties in a comfortable familiar environment and perform at their peak.
This all happens best in a familiar environment for the plastic surgeon working with the same team and the same anesthesiologist as well as the same instruments. The results are more predictable and the outcome more beautiful. Being able to perform cosmetic procedures in a certified operating room with excellent staff often allows for a better experience for the patient and in our minds a better result. Plastic surgery offers the powerful chance to change the appearance. This opportunity provides hope to those afflicted by accidents and unforeseen injuries that often disfigure the appearance. It also offers those with congenital anomalies the opportunity to alter whatever they were born with into something that they desire. It allows people to choose their own appearance. It allows one to rejuvenate their body and remove unwanted excess or attain a little more in areas making clothes fit better or some to feel beautiful. Plastic surgery provides hope. The challenge with this opportunity is often that the expectations of what can be achieved often exceed the possibilities of what surgery can provide. In some circumstances the expectations can even exceed what can be done in multiple surgeries.
This is a difficult situation and plastic surgeons as well as some cosmetic surgeons will often refuse perform a procedure if after consulting with a patient it still appears that the expectations exceed the normal or expected outcome of surgery. It is important to also make sure that the patient understands that surgery has many unknowns and results vary widely based upon the individual’s own tissue and healing process. No two people are identical and no two healing processes are identical. This creates the possibility of variance and unpredictable results. If one cannot handle variance cosmetic or even plastic surgery may not be right for you.
Before having plastic surgery it is important to remember that a touch up or revision surgery is a real possibility and that it will cost more to do such. Many people choose to have additional surgeries when the outcome of the surgery is not what they anticipated or when their expectations change after the healing process and a little more enhancement is desired. This is a slippery slope as one usually cannot be too beautiful, too young, or have too much money. We suggest that once a decision is made and a procedure is completed that one waits six months to a year before considering any more changes to the appearance as this feeling of wanting to do more is common and perfection is not attainable.
Remember that the healing process also changes your mood. The healing process will create some anxiety and can result in a depressed mood state. This can often be the most difficult part of the recovery process. We see this most commonly for larger surgeries such as abdominoplasty and mastopexy or breast lift as well as for facelifts and blepharoplasties. It is important to remember that the mood and the anxiety pass as the healing completes and your new look emerges.
It is important to consider the effect healing and the appearance transformation will have on your mind prior to surgery and develop a plan to deal with these predictable responses to surgery and changing expectations. We have observed that having a spouse, partner, significant other, or friend that remains positive and supportive throughout the healing process often results in a more peaceful recovery and in overall better results. At Chrysalis Cosmetics we spend time prior to your surgery going over your expectations and expected results. We also counsel about the known unanticipated results and all the options for handling the situation. This pre-transformation educational process is done in a verbal and written format so that you have opportunities to go over this prior to any change of your physical appearance. We feel that while your physical appearance changes your inner self changes as well and this is often the more challenging part of the transformation process for our patients. Otoplasty or surgery of the ear is something that can have a dramatic effect upon a person’s life. Having malformed or very large ears creates much unwanted attention for the owner. What we see after fixing the ears or pinning the ears is truly amazing. To witness the expression one has after having their ears fixed is phenomenal. Large ears affect not only one’s hairstyle but can affect confidence as well, especially if they are subjected to teasing. Plastic surgeons have special training in helping people with unique characteristics make changes that allow them to appear like they wish. (Notice the left ear before incision-less otoplasty surgery and after below)

At Chrysalis Cosmetics we have started performing the incision-less otoplasty for some patients. Incision-less otoplasty has actually been around since the 1960’s however was done from the front of the ear. Dr Charles Perry places sutures to form the antihelical fold that is often missing in children and adults with large ears. By combining several well placed sutures and anterior scoring of the antihelical fold cartilage a fold can be created. We are finding this procedure is well tolerated and creates a natural looking ear with less recovery time. The plastic surgical procedure takes about an hour. Of course if the conchal bowl (or cup of the ear) is too large a set back or resection of the excess cartilage may be required and does require an incision and slightly more time. Of course a thorough examination is required prior to any decision as to what technique can be used to reduce the prominence of the ears and a discussion with your plastic surgeon as well. Breast augmentation using silicone breast implants has become our most popular procedure over the past year here at Chrysalis Cosmetics in Sacramento California near Roseville and Stockton. It is really not surprising as silicones make up 90% of the earth’s outer crust. Silicone is found in almost everything we use (lotions, shampoos, deodorants, toothpastes) usually as dimethylsilicone silica, silica dimethyl silylate, and cyclopentasiloxane. One of the reasons for silicone being used so readily is its abundant supply, it variety of forms and uses as well as its safety profile. Silicone exists as a solid, a gel or even a liquid depending upon the number of bonds it has with its neighboring silicone or other element. Because silicone is so malleable, a cohesive gel has been created that is approved for use in humans that feel very similar to breast tissue. Cohesive gel is a term meant to describe a state between a gel and a solid.


It would make sense that silicone would work as a synthetic breast as you would want something with the feel of tissue that could move with your normal tissue but would maintain its shape and stay together if its shell broke.
The only silicone breast implants that are available now for implant are either solid or gels. Most women prefer a (cohesive) gel silicone implant for their breast augmentation and most men prefer a solid silicone implant for their pectoralis muscle augmentation. The only silicone gels now available in the Unites States are cohesive gels – soft, mobile yet even if the outer protective shell of the implant breaks (and it can over time from oxidation) – the inner gel will stay together until removed by your plastic surgeon. 
In the 60′s and 70′s the first two generations of silicone implant had a gel that was not as cohesive and as such it ended up spreading throughout the breast tissue and into the lymph nodes. Prior to 1960 some people were just injecting silicone (meant to lubricate machinery) into the breast. As such this impure silicone caused a vigorous tissue response and often created breast that were much distorted over time and the droplets were difficult to remove. Many women required mastectomies to completely remove the micro droplets that often migrated throughout the breast and chest. Since then silicone is now medical grade, pure and more cohesive. In the near future we may see a variety of cohesive gels that vary in their firmness with some being almost form stable (like a gummy bear or a jello).
In the implant you choose for enhancing your breast is a very personal choice. This is why it is so important to review the choices of breast implants prior to your consultation or surgery. At Chrysalis Cosmetics, Dr Perry will examine you and help you make an informed decision on what implant is right for your breast augmentation.
The surgical technique for placing your silicone or saline breast implant is similar however the incision is slightly (2 cm) longer when placing a gel implant as it is filled prior to placement and the saline is filled after placement. I recommend silicone implants for women who want a more natural look and feel without clothes on. I hope this is helpful.
