More Women choose Gel implants for Breast Augmentation

Sacramento California. One of the interesting trends we are witnessing this year is the resurgence of plastic surgery procedures such as breast augmentation.  The American Society of Plastic Surgeons released statistics from 2010 taken from its members for the year 2010.  The survey revealed an overall  5 % increase in plastic surgery procedures compared to 2009 – which we found surprising given the state of the economy.   However it seems that people are moving forward with their lives that had been put on hold and feeling more confident about their future.  In regards to breast augmentation an increase of 2% (296,000) was witnessed.  The great majority of women choosing to enhance their breast are choosing to do so with silicone gel implants as well with over 60% of women choosing this implant style for its natural feel despite it being slight more expensive.   One of the recent advances in silicone gel implants for breast augmentation has been the increased use of a placement device – The Keller Funnel.  This has allowed plastic surgeons to place the gel implant more gently and perhaps decrease the risk of bleeding infection or scar tissue in the future.  Prospective studies will be needed to confirm these claims moreover it remains a plastic surgeons choice and methods vary.  Breast enhancement and breast enhancement revision remain popular procedures.   We look forward to another exciting year.

Plastic Cosmetic Surgeons Treating Crows Feet with Botox, Browlifts, and Fillers

One of the exciting advancements in the past decade has been the realization that as we age our face looses volume in specific areas.  The cheeks, below eyes and in the temporal hollow just lateral to the eye towards the hair line are three examples of these deflationary areas.   The lateral temporal hollow thins as we age and occasionally after facelifts or brow lifts.  When thinning of the lateral forehead occurs the skin droops down and the lateral brow drops towards lateral eye – crowding this area.  For years we have been successfully using Botox or other chemonueromodulators such as Dysport to relax muscles around the eye to let the brow elevate and smooth the crows feet– however this does not address the ultimate reason for the extra skin.  In the past we also tightened the skin as well.  Removing the excess created by the deflation of the lateral temporal hollow. Endoscopic brow lifts work well however may not be right for some. We have also found that there is a limit to how many facelifts or browlifts one can have before over tightening the skin and creating an altered look.  Fillers such as structural fat grafting have allowed us to maintain a more natural volumized look.  We have found that Juvederm or Restylane tend to work well to fill the temporal hollow and lift the lateral brow and shape the forehead.  It is a softer look that has made many patients very happy.

Risks of the injection are similar to other areas of the face including bruising and swelling as well as pain to list a few.  This is also an off label FDA usage of the products as they are approved for use in the folds of the face such as the nasolabial fold around the mouth.  Other products such as Sculptra and Radiesse also work however these are more permanent and difficult to adjust after placement.  Structural fat grafting is also an option and is a more permanent solution.  All of these options exist so that you and your plastic surgeon can create a designer plan to fulfill your individual goals.   Facial cosmetic surgery is a very exciting field of medicine currently with many new ideas and options to assist you in continuing to look your very best.

Plastic Surgery In The United States; Staying Home – Worth The Cost

It is interesting to see so many Americans traveling abroad to have their cosmetic surgeries performed. It is also troubling to read blogs encouraging people to travel for their surgery citing one example of a succesful outcome. Its like knowning someone who won the lottery. We certainly realize that it is cheaper to have plastic surgery in other countries, however, cheaper is not always better and the risks you take may be quite high. The real question is will the cost savings (often less than a $1000) be worth the hassle and extraordinary risks as well as the travel arrangements and extra time away from home?
Only you can answer that. Let us consider why staying close to home in a familiar and safe setting with board trained physicians may actually be worth a few extra dollars especially when it comes to your health and body. Rhinoplasty Surgery Advances With the FDA Release of a New Flexible and Absorbable Scaffold for Plastic Surgeons

The Flexible and absorbable PDS plate assists your rhinoplasty surgeon in achieving a straighter nose following trauma, nasal fracture. It has also been utilized to assist in strengthen the cartilage grafts sometimes required to make a pretty nasal tip or when removing large humps from the bridge of the nose. The septal or ear cartilage grafts utilized by your rhinoplasty surgeon when creating your new nasal contour can be secured or fixed to the plate allowing for greater strength – like a batten. Certainly we want to exceed the standard of care in a way that is comfortable, sustainable, affordable and safe. While we have regular internal audits devoted to improving ourselves we also find it helpful to have our facility audited by an outside agency, IMQ. IMQ is a division of the California Medical Association that certifies facilities that perform surgeries. Our processes are compared and bench marked for your safety. A full day of surgery and clinic was recently observed as were our internal processes and procedures. It requires extra effort from our staff, who always rise to the occasion. Plastic Surgery Following Weight Loss in California

Plastic Surgery in Sacramento California has seen an increase in post weight loss surgery. This is sometimes referred to as MWL (Massive Weight Loss). Any weight loss over 100 pounds is usually considered to be massive however there is certainly variablity as to the amount that is lost and that is referred to as massive. Cosmetic plastic surgery following MWL or post bariatric (referring to surgical procedures that result in significant long term weight loss) procedures require a comprehensive plan. Plastic Surgeons across America have begun the process of developing specific techniques that allow transformations following MWL. Dr Charles Perry of Sacramento California utilizes three procedures to transform the body as a unit. At Chrysalis Cosmetics we recommend starting from the top down as these areas are most visible to others. Starting with rejuvenation of the neck, eyes and face is usually suggested as these areas of the body are less concealed by clothes and are seen by others the most. A brow lift opens up the eyes and removes the tired look often associated with MWL. Blepharoplasty removes the excessive skin around the eyes and brightens and enhances the appearance – often in a dramatic fashion. Brow lift and blepharoplasty combined with a facelift, which removes the excess skin hanging from the neck and restores volume to the cheeks, improves the overall facial balance and beauty while removing the constant reminder of having lost large amounts of weight.

After restoring facial elegance many desire removal of the apron of skin hanging from the body and arms. Brachioplasty combined with a breast enhancement (referred to as an upper body lift) releases the upper body from the constant reminder of MWL and certainly improves both the appearance and spirits for many. The breast and arms or chest and arms for men are often a source of embarrassment and serve as a constant reminder of previously being large. Once the excessive skin is removed the appearance improves the ability to wear more flattering clothes allows one to show off the new beautiful body that has completed a life changing transformation. An upper body lift combined with a lower body lift is often the most dramatic. The excessive skin often referred to as a pannus hanging from the abdomen and flanks as well as the thighs can be removed by pulling the thigh skin up and the abdominal skin down. This pushing of the abdominal skin down and the pulling of the lateral thigh skin in an upward direction greatly enhances the appearance of the thighs, hips and abdomen. Enhancing the appearance of the abdomen often allows one to feel healthier as well as fit into clothes easier and greatly improves the ease of self hygiene. It is amazing to see the excitement and joy people experience when the final reminder of weight loss is removed and their transformation becomes complete.

Other procedures that may improve the appearance following MWL include a medial thigh lift, buttock augmentation, breast augmentation, rhinoplasty (nose lift) and on occasion some liposuction. However liposuction is usually unnecessary as following MWL most have excess skin that just requires removal and adjustment to enhance the contour.

We have noticed that many people around the Sacramento area are enhancing their smiles with new whiter teeth and perfect lips. Seeing facial harmony in a smile with smooth skin, straight pretty teeth and full lips and cheeks can be quite beautiful. However if the skin doesn’t match the teeth it can look quite unnatural. You may have had this experience already seeing bright straight teeth framed by loose wrinkled thinned skin. The contrast can be quite overwhelming and distracting from the smile. Bright smooth skin and beautiful lips can compensate for less than white teeth however the opposite is usually not true.
What constitutes a beautiful smile is varied, however, classically it involves upper lip elevation, straight beautiful teeth, perhaps a little head tilt and some elevation of the cheeks with some widening and narrowing of the eyes and perhaps some nose crinkling with some brow descent. The key is that all the components match and some symmetry exists.
Aging can affect the smile, and the lips in particular are an extremely sensitive area. One of the aspects of age is thinning and loosening of the skin. Whether from sun damage, smoking or hyperactivity, this dermatohelosis occurs to some extent in all of us. We see this around the eyes – crow’s feet, between the eye brows – elevens, or around the mouth (the parenthesis defect) or even necklace lines. Around the lips this process often results in lip lines referred to as smoker’s lines. What makes the lips unique is their vigorous attachment to the underlying muscle. This muscle (The Orbicularis Oris) moves when we talk eat and pucker. The attachment of the skin to the muscle creates a valley as the skin thins and the loose skin creates a peak between the attachments as the thinning of dermis occurs and the elasticity of the skin decreases. With those who are thin with very active lips or for those with sun damage the lines will be more apparent.
One of the newer treatments we have added for the lips (perioral area) at Chrysalis Cosmetics is the buff and puff. The buff is the laser resurfacing and the puff is the Hyaluronic acid filler. Using injectable fillers such as Restylane or Juvederm we are able to puff the line up by filling the valley so to speak. The filler brings back that fullness and softness of youth and health. Using the Tunable Sciton Laser’s erbium beam we are able to soften the peak of the lip line and stimulate new brighter fresher skin to grow and fill in the valley and soften the peaks, buffing the irregularity. The laser can be tuned to your own personal skin type and wrinkles and go no deeper than necessary, virtually eliminating the risks for hypopigmentation once seen with CO2 lasers around the mouth.

The Brazilian Buttock Lift Enhancement in Sacramento California for Fuller Rounder Buttocks

Full, round buttocks are often viewed as sign of beauty and vitality. Magazine covers and TV programs often emphasize the fuller buttocks. While Cosmetic Plastic Surgeons have been able to augment the breast they haven’t until recently been able to change the shape of the buttocks. The silicone implants didn’t give enough volume and often became hard or migrated out of place.

One of the great advances in plastic surgery over the past 20 years has been the advent of fat transfer. Following liposuction to collect the excess or unwanted fat cells they are then transferred from one area of the body to another. Structural fat transfer has become much more common and more reliable. With liposuction and fat transfer we can make the waist smaller and the buttocks rounder. This procedure often referred to as a Brazilian Buttock Lift. We like to refer to this as a buttock augmentation through structural fat transfer. The procedure is tailored to the individual characteristics and optimized for the individual’s goals. Recently we had one such person whom desired a rounder full buttocks. She had thought about this for years and finally decided to something about it. A transfer of 400 cc of fat from the abdomen to the buttocks was performed! These are her before and after photos. We are so excited for her. Her pant size has gone up by 2 sizes!
